In Bridgeton, MO, Hussman Corporation lost a worker when he was pinned between a scrap metal table and a railing.

The company, after being investigated by OSHA, was cited for 15 violations, 3 considered willful and 12 considered serious.

According to the area OSHA director, this accident could have been prevented.

The three willful violations include lack of lockout/tagout procedures on some equipment while not correcting problems in the current LO/TO procedures on other equipment.

The twelve serious violations include lack of training for employees, unsafe practices related to powered industrial trucks, improperly closed electrical cabinets, and using damaged electrical cables. Proper workplace electrical safety training prevents these types of violations.

As a result of the investigation, the company has been placed on OSHA’s Severe Violator Enforcement Program, and the proposed fines exceed $272,000.
